Liability Waiver, Express Assumption of Risk, and Release of Liability

Child Parent Connection, A Family Therapy Corporation (DBA: South Bay Mommy and Me), provides a peer-to-peer forum for discussion where parents can share thoughts, ideas, and concerns.  The classes provided by South Bay Mommy and Me are led by licensed psychotherapists and mental health professionals; however, these classes are not intended to be a substitute for the advice and/or treatment by your health care providers.  Furthermore, the needs of each infant and child are unique, and no service or forum provided by South Bay Mommy and Me is intended to replace the advice of a healthcare and/or childcare professional.

 

South Bay Mommy and Me is a rented facility.  Despite the high standard of care and level of attention to detail to ensure safety standards for you and your child, it still remains possible that during your participation in any of our classes or workshops, both you and your child may be exposed to a variety of unforeseen hazards and risks.  These risks include, but are not limited to serious personal injury and property damage (“Injuries and Damages”) caused by the condition of the rented premises.  You understand and acknowledge that you may have to exercise extra care for you and your child.  You further understand that South Bay Mommy and Me does not have rescue or medical facilities or the expertise necessary to deal with any Injuries and Damages.

You further confirm that:

  • You are the parent and/or lawful guardian of the child.

  • You consent to the participation by the child in all activities conducted by South Bay Mommy and Me.

  • ACTING ON BEHALF OF YOURSELF AND YOUR CHILD, AND TO THE EXTENT ALLOWED BY LAW, YOU HEREBY WAIVE AND AGREE TO RELEASE ANY CLAIMS OR LIABILITY WHICH YOU, YOUR CHILD, OR YOUR CHILD’S HEIRS AND SUCCESSORS MAY HAVE OR BELIEVE TO HAVE AGAINST South Bay Mommy and Me AND ITS OFFICERS, EMPLOYEES, OR AGENTS FOR ANY INJURIES, LOSSES OR DAMAGES TO YOUR CHILD, YOUR CHILD’S PERSONAL PROPERTY AND YOUR PERSONAL PROPERTY.   

  • YOU AGREE TO BE RESPONSIBLE FOR, INDEMNIFY, AND HOLD HARMLESS South Bay Mommy and Me FROM AND AGAINST ANY CLAIMS, SUITS, JUDGMENTS, OR COSTS WHICH MAY BE BROUGHT AGAINST South Bay Mommy and Me, ITS OFFICERS, DIRECTORS, EMPLOYEES OR AGENTS FOR THE ACTUAL OR ALLEGED ACTS OR OMISSIONS OF YOU OR YOUR CHILD.

 

I have read the above Liability Waiver, Express Assumption of Risk, and Release of Liability in its entirety and I freely and voluntarily assume all risks of such Injuries and Damages and notwithstanding such risks, I agree to participate in the classes at South Bay Mommy and Me.
